History and Problems of Concept of Positive and Negative Symptoms in Schizophrenia

Abstract
In this paper, the author reviewed the historical background and unresolved problems of the conception of the positive and negative symptoms in patients with schizophrenia. In the past several decades, the distinction between positive and negative symptoms in schizophrenia has been issued in the fields of clinical research and practice. Even though the concept of positive and negative schizophrenia had been emerged from the end of 19th century, the issue of reliability and validity of these symptoms appears to have not been resolved completely. The dichotomy of schizophrenia in two kinds of disease-positive and negative schizophrenia-needs a critical review. Despite the popular usage of these paradigm, the results of many studies had remained to be indistinct. This is largely due to poor validity of the scales using evaluating these symptoms. The improvement of the methodology of defining symptoms in schizophrenia has made possible to enhance the efficacy of clinical practice and psychiatric research. Today we are not able to determine the definite positive and negative symptoms, so new refining studies are needed.
